Driving While Intoxicated (D.W.I) Evaluations

Court-Ordered or Attorney Recommended Evaluations

If you’ve been arrested for or convicted of an impaired driving offense in NYS, we can provide confidential and state-approved evaluations to support you to meet your legal requirements and move forward.

What Does a DWI Evaluation Entail/Include?

Once an evaluation is complete, you will be informed whether or not further treatment would be recommended. Further treatment could be through a drug/alcohol detox program, inpatient rehabilitation, or outpatient treatment and education. 

An Evaluation Includes:

  • Full mental health and substance abuse assessment, which can take between 1-3 sessions to complete. (in person only)

  • Sign 4 HIPAA Release of Information Forms:

         (1) Signed NYS OASAS Impaired Driving Services Consent Form

         (1) for lawyer / court

         (2) to allow providers to gather information from other collaterals (family member, friend,                       boss, doctor, previous providers

  • Arrest Report and supporting documentation

  • *Complete a urine toxicology at LabCorp (or your preferred lab) with requisition received by    provider completing assessment.

*The fee for the toxicology is included within the fee for the DWI evaluation IF completed at Labcorp. You will be provided with the information to schedule when your evaluation is scheduled and the fee is paid. If you prefer to complete the toxicology elsewhere, you will be responsible for scheduling/paying for the toxicology, as well as providing the results to the provider. Please note that a urine toxicology is required for the DWI evaluation and it cannot be complete without it. 

If all documentation is submitted at least 3 business days prior, your evaluation can typically be completed in one session

Steps to complete you evaluation:

  1. Schedule your evaluation

  2. Obtain your intake packet and complete all forms

  3. Submit your documentation

  4. Attend your evaluation (in person)

Fees & Expedited Option

Total Cost: $350.00 (non-refundable, required at booking to hold your appointment) (Debit/credit card (via Sessions Health or Ivy Pay accepted)

Expedited Evaluation & Report: +$100.00 (For clients with upcoming court hearings within 1-2 weeks)

*Evaluations are not covered by insurance

Late or Missed Apts: If you are late (over 15 min) for your apt, it will only be rescheduled one other time.
